This happens when a changeset has been pushed to Kiln but no case number was put in the commit notes. So I would go to the changeset and manually add it.

As I mouse over "Cases (0)", it changes to "Add a case". Clicking this doesn't seem to do anything. If I use Firefox it shows a small box underneath which allow details to be entered. If a changeset already has a case linked to it, then this shows a different screen layout which does work in Chrome.

Edit: As pointed out by M. Esh in the comments, this is not an issue with Chrome 7.0.517.41 with any of the extensions enabled as mentioned in the accepted answer below.

That's strange. I just tested this with the same version of chrome, and it appears to be working for me. Here's a weird question for you, what happens if you click and move the mouse slightly? Does it appear then? – Jason Rosoff Oct 8 2010 at 14:25
I've just tried what you said but it still doesn't appear. Even stranger though, "Add a Tag" and "New Review" do work. – Paul Healey Oct 11 2010 at 9:07
confirmed this issue for our test install too. Works fine in IE7, does not work in Chrome, 6.0.472.63 – M. Esh Oct 11 2010 at 16:07

Thanks to help from customer support, specifically Bradford, we worked out that it was a Chrome extension:

Disabling it made "Add a case" work. I have an On Demand account as well and strangely it worked regardless there. It only seemed to effect our On Site install.

Also, Bradford did suggest the following workaround for anyone having the issue:

...when it changes to "Add a Case" can you right-click and tell it to open in a new tab? You should get a page with a form asking you to associate a changeset with a case. Admittedly, it's not as fancy as a pop-up box, but it should do the trick!


Edit: I think it might be related to mouse gesture related extensions in Chrome since this one stops it working as well:

why would the on demand account work OK, and the On Site install not work? I'm not at work right now, but I'm pretty sure my only extension in Chrome was RSS. – M. Esh Oct 14 2010 at 1:54
